McMan is recruiting for a Case Manager to join our team providing a housing first and harm reduction approach; the Case Manager will be responsible for assisting residents in maintaining their housing, assist accessing financial supports, supporting their education/employment goals and any other areas of support that is required to help them successfully transition into independence. The Case Manager will work alongside our Transition Specialists, by being a member of the resident’s team that is planning around the needs for their successful transition into the community.

 

 

*This role will require on call support on a rotating basis and some evening and weekend work hours.
