The Youth Transition to Adulthood (YTA) program provides individualized support to youth to help them prepare for independence. We are currently seeking someone who has a passion for working with youth to join our YTA program as a Transition Worker. The Transition Worker is responsible for supporting youth to locate appropriate housing options based on their needs, build their capacity and self-efficacy, make informed choices and plan for their futures, work towards their education and employment goals, access community resources and services, increase their social connections and build a stronger support network. 
 
The Transition Worker will also be actively involved in supporting youth to address various complex needs including addiction and mental health and teaching life skills that will allow the youth to transition successfully into independence.

*This position requires a reliable vehicle, the ability to work a flexible shift pattern that will include some evening/weekend work, and the ability to participate in the organization's on-call rotation*
 
Some of the key aspects of this role include:
  • Supporting youth in learning skills required for successful independence, accessing community resources, maintaining their health and well-being and engaging in school and employment.
  • Helping youth navigate mental health and justice systems
  • Identifying and engaging natural supports
  • Understanding needs related to pregnancy, childbirth and parenting
  • Locating and supporting youth with maintaining safe and affordable housing
  • Providing support and coordination for life skills groups and recreational activities
  •  Working with youth to develop safety plans
 

The ideal candidate will have:

  • Diploma or a Degree in Human Services, Child Studies or a Child and Youth Care Counsellor Degree (CYCC) is preferred
  • Experience working with youth with complex needs
  • Knowledge of the Natural Supports and Trauma Informed Practice Frameworks
  • Demonstrated ability to facilitate relationships and connections where youth expand their personal support networks and find new and meaningful relationships
  •  Demonstrated skills in crisis intervention, conflict resolution, critical thinking and problem solving, as well as written and verbal communication skills
  • Knowledge of appropriate community resources
  •  Current standard First Aid Certification and Suicide Intervention Training
  •  Current police check and Child Intervention Record Check (current within six months)
  • Valid Driver’s license, adequate insurance coverage (2 million dollars 3rd party liability) and own transportation

Salary Range:   $49,768.00 to $54,931.00
