McMan is recruiting for a Part-Time Transitions Specialist (Education and Employment) to join our team. The Transitions Specialist ensures services effectively address the most relevant barriers young people encounter in achieving long-term housing stability. The Transition Specialist assists in connecting youth to educational resources, facilitates workshops on life skills, financial literacy, and job readiness, and provides support and mediation to strengthen youth's natural supports and shared goals in their transition to independence. Key responsibilities include building partnerships among service providers and local businesses to create a comprehensive network for youth access to employment and financial supports.
KEY RESPONSIBILITIES
- Manage a fluctuating caseload based on the need of the youth in program
- Support youth voice and choice in setting employment and education goals while understanding the stages of change
- Assess and support by identifying each client’s strengths, needs, abilities, and preferences
- Design and implement programs that address barriers to education, employment, and financial stability for youth
- Facilitate workshops on life skills, financial literacy, and job readiness to equip youth with essential competencies for independence
- Build and maintain relationships with service providers, educational institutions, and local businesses to create a network of employment and financial support opportunities for youth
- Deliver intervention and support services, advocacy, and practical assistance through one-on-one work, collaborating with program Case Managers to achieve transition outcomes
- Utilize a Natural Supports approach to enhance clients' support networks
- Provide transportation for youth and their supports in a personal vehicle, as related to goals
- Represent the program at community events and collaborate with employment agencies, employers, educational institutions, and other partners to connect youth with opportunities
IDEAL CANDIDATE
- A Diploma or Degree in Human Services
- 4 years of relevant work experience
- Strong interpersonal skills, advocacy skills, program planning and administrative abilities
- A good knowledge of community resources
- Valid Driver’s license and $2million liability insurance
- Valid First Aid Certificate with CPR and Suicide Intervention Awareness Training (ASIST)

Salary Range: $28,692.00 - $31,880.50
